Transfer Medical Records (From any Clinic)

In order for us to obtain your previous medical records, you will need to do the following:

  1. Download, fill out and sign the following form: Medical Records Transfer.pdf. You can either print the PDF and manually sign, or electronically sign the PDF (to electronically sign, you will need to download and open the file in Adobe Acrobat Reader DC).

  2. Once completed, you will need to email, fax, or mail the form to your previous medical provider. Its always a good idea to call ahead and verify how they will receive the form. Each provider has a different process for how they will accept a request for transfer of medical documents.
